Required documents must be available inside the patient file:
Insurance paper + National ID or Passport

Detailed Medical report including:
- Name of operation and diagnosis
- Cost of operation / admission
- Reason of operation / admission
- Duration of hospital stay
Discharge form stamped and signed by the hospital manager (if admitted; in case of operation or hospitalization)
Lab results + Histopathology report if available (stamped and signed by lab supervisor or histopathologist) (Original is required)
Radiological imaging films (with name and date mentioned on the imaging) along with their reports (Original papers are required) (stamped and signed by the radiologist)
Hospital bills (Original) (Stamped and signed by the Hospital manager and Hospital finance manager)
Pharmaceutical bill (Original) (Stamped and signed)
Pictures of the prescribed medications
Birth certificate document for C-section or normal delivery
